Restaurant feedback questions for a QR code on the table
A comment card gets filled in by the happiest and the angriest guest of the night. A QR code on the table, with one question that takes five seconds, gets everyone in between.
Ask one question per table tent and rotate it weekly. Guests answer a single tap far more often than a ten-question form, and you learn something specific each week instead of a vague average.
How to run it
- 01Pick one question for this week and publish it as a printed poll.
- 02Print the table tent format, fold it, and put one on every table.
- 03Check the results page on Monday, then swap the question.
The questions
Each link opens the editor with the question already written. Change anything, then publish and put the QR code on screen.

Tips from running these live
- One question per tent. The second question halves the answers to the first.
- Put the QR code where a hand naturally rests: next to the salt, not on the window.
- Say what you changed. A small sign that reads 'You asked for more vegetarian options, here they are' makes the next poll twice as popular.
